Part 1: The Core Requirements

Book the Appointment: Ensure you have booked a formal appointment at your designated PICA office or Embassy/Consulate jurisdiction.[cite: 8]

The Official Form: Complete the Jamaican Passport Application Form in blue or black ink. Do not use white-out.[cite: 8]

The Signature Box Warning: Do not sign the biometric signature box at home. Leave it empty until you are standing directly in front of the officer. The signature is scanned by a computer; if your pen touches the boundary lines of the box, the machine will reject it.[cite: 8]

Part 2: The Physical Documents

Proof of Citizenship: Your original Jamaican Birth Certificate, Adoption Certificate, or your physical Certificate of Jamaican Citizenship. (No photocopies or laminations).[cite: 8]

Valid Photo ID: An original, valid government-issued ID (e.g., Driver's License, National ID, or another country's passport).[cite: 8]

The Application Fee: Check your specific location's fee. Consulates require a Certified Check or Money Order. PICA Jamaica accepts cash, debit, or credit.[cite: 8]

Part 3: Photographs & Certification

The Photographs: Two identical, color passport-sized photographs taken on a matte finish against a plain background.[cite: 8]

Consulate Overseas Rule: If visiting a Consulate Overseas, do not have a Notary sign your photos or Section G in advance. The Consular Officer will officially certify them during your appointment.[cite: 8]

PICA Office Local Rule: If visiting a PICA Office in Jamaica, you must have an eligible certifier (e.g., a Justice of the Peace) sign Section G and the back of one photograph before you arrive.[cite: 8]

Proxy & Mail-In Eligible: If you already hold an adult Jamaican passport and need to update your legal name due to marriage, divorce, or a Deed Poll, you do not need to appear in person. You can authorize DocuHub Operations Services to submit this application at the PICA headquarters in Jamaica on your behalf.[cite: 8]

Part 1: The Core Identity & Name Change Documents

Proof of Citizenship: Your original Jamaican Birth Certificate, Adoption Certificate, or Certificate of Jamaican Citizenship.[cite: 8]

The Bridge Document: You must submit the original legal document that proves the name change (e.g., Original Marriage Certificate, Deed Poll, or Divorce Decree). PICA will return this original document to you with your new passport.[cite: 8]

The Current Passport: You must submit your current Jamaican passport so it can be officially canceled and replaced.[cite: 8]

The Official Form: Complete the standard Jamaican Passport Application Form using your new legal name.[cite: 8]

Part 2: The Proxy Protocol (DocuHub Submission)

Because you are not appearing in person, your application must be heavily certified before you courier it to our office.[cite: 8]

Notarized Authorization Letter: A formally typed letter stating: "I, [Your Full Name], authorize DocuHub Operations Services to submit my passport renewal and name change application, and to collect my new passport and original supporting documents on my behalf." You must sign this in the physical presence of a Notary Public, who will then stamp it.[cite: 8]

Fully Certified Application: A Notary Public in your home country must sign and stamp Section G of your application form.[cite: 8]

Certified Photographs: Provide two passport-sized photographs. The Notary Public must certify the back of ONE photograph.[cite: 8]

Secure Courier Shipment: Courier your complete package (Original Birth Certificate, Current Passport, Original Bridge Document, Certified Form, Photos, Notarized Letter, and Application Fee) directly to the DocuHub office via a secure, tracked service like FedEx or DHL.[cite: 8]
