01 First-Time Adult Application (18+)

If you have never held a Jamaican passport as an adult, or if your last passport was issued to you as a minor, you must apply in person to officially establish your legal identity. This process strictly requires original physical documents and an Authorized Official to verify who you are.

1 Gather Your Documents

  • Blank Application Form: You can download and print this directly from the official PICA website.
  • Original RGD Birth Certificate: Must be the physical original, not a copy or laminated.
  • Valid Government Photo ID: Original Voter's ID, Driver's Licence, or National ID. (Note: School IDs are not accepted).
  • Printed Passport Photos: Two (2) professional physical photos.
  • Marriage Certificate: Original document (only required if you are currently or have been married).

2 Complete the Application Form

    Applicant's Personal Data: Full current legal details exactly matching your birth certificate.
  • B
    Marital Status: To be completed if you are or have been married.
  • F
    Emergency Contacts: Two (2) emergency contacts required.
CRITICAL STEP: Do not sign the Applicant's Declaration (Section E) yet! You must sign this section directly in front of an Authorized Official (like a Justice of the Peace or Police Officer). The official will watch you sign, then they will complete Section G and officially certify the back of one of your printed photos.

3 Your Final PICA Submission Pack

Once signed and certified, put these items together to take to the PICA office (or hand over to your Docu-Hub proxy bearer):

  • Completed Application Form (Sections E & G signed/sealed)
  • 1 Certified Photo + 1 Regular Photo
  • Original Valid Photo ID
  • Original RGD Birth Certificate
  • Original Marriage Certificate (if applicable)
  • Payment Method (Visa/Mastercard/Debit/Cash)

02 Adult Passport Renewals

If you already hold an adult passport, the requirements change drastically based on your chosen submission method. Choose the 100% Online Method for direct renewals, or the In-Person Method if your name has changed or you wish to use a physical proxy bearer.

A Option A: The 100% Online Portal Method

You do not need to print any physical paper forms for this. Before going to the PICA website, gather these specific items:

  • Eligibility Check: Your current passport must have been issued after September 2001, and your legal name must not have changed.
  • Active Email Address: You must be able to log into this email immediately to access the portal.
  • Current Passport Book: Keep it handy so you can type in your old passport number and issue date.
  • Birth Certificate Details: You will need your Original RGD Birth Certificate to locate and type in your birth entry/certifying number (usually located on line 3).
  • Digital Photo Upload: A digital image file formatted exactly to PICA's online background and size rules.
  • Specific Personal Data: Have your exact date of birth, mother's maiden name, and current residential address ready for delivery/pickup options.
  • Two (2) References: The full names, professions, complete physical addresses, phone numbers, and emails for two contact persons.
  • Payment Method: A valid Visa or Mastercard to process the government fee online.
The Portal Process:

Go to the PICA website. The portal will take you through an Eligibility Checklist to confirm your qualifications. Once you pass, you will type in your personal data, upload your digital photo, choose your delivery method (Pickup or Courier to your current address), and enter your card details to finalize the submission.

B Option B: In-Person Physical Method

If your legal name has changed due to marriage, divorce, or a Deed Poll, or if you simply prefer to submit physical documents in person or via our proxy bearer, you must use this method.

2. PICA Document Expectations
  • Current Passport: Must be physically surrendered to PICA.
  • RGD Birth Certificate: Original physical document.
  • Printed Photos: Two (2) physical photos. (If Name Change, ONE must be certified on the back).
  • Legal Bridge (Name Change Only): Original Marriage Certificate, Decree Absolute (divorce papers), or Deed Poll.

03 Adult Passport Replacement (Lost or Damaged)

If your passport has been washed, torn, mutilated, lost, or stolen, it cannot be renewed online. PICA mandates a strict physical application process and specific legal declarations to authorize the replacement and protect you against identity theft.

1 Gather Your Documents

  • Blank Application Form: Downloaded and printed from the PICA website.
  • Original RGD Birth Certificate: Must be the physical original.
  • Valid Government Photo ID: Original Voter's ID, Driver's Licence, or National ID.
  • Printed Passport Photos: Two (2) professional physical photos.
  • IF YOUR PASSPORT IS DAMAGED: You must have the physical damaged passport book (regardless of its condition) AND a written affidavit detailing exactly how the damage occurred.
  • IF YOUR PASSPORT IS LOST OR STOLEN: You must have an Original Police Report (officially signed and stamped by the station).

2 Complete the Application Form

Note for Damaged Passports: While you are at the JP, you must also have them properly sign, stamp, and notarize your written damage affidavit so it is accepted by PICA.

3 Your Final PICA Submission Pack

Take these items to the PICA office (or hand them to your Docu-Hub proxy bearer):

  • Completed Application Form (Sections E & G signed/sealed)
  • 1 Certified Photo + 1 Regular Photo
  • Original RGD Birth Certificate
  • Original Valid Photo ID
  • Proof of Circumstance: The Damaged Book & JP-Notarized Affidavit (If Damaged) OR The Police Report (If Lost).
